Participation Waiver & Parent/Guardian Consent

I am the parent or legal guardian of the athlete being registered and give permission for my child to participate in Showtime Youth Performance. I understand that athletic training and physical activity involve inherent risks of injury.

I confirm that my child is physically able to participate and agree to inform Showtime Fitness of any relevant medical condition, injury, allergy, medication, or limitation.

In the event of an emergency, I authorize Showtime Fitness staff to obtain appropriate emergency medical assistance for my child if I cannot be reached promptly.

I understand the program schedule, participation requirements, and registration fee. I also understand that Showtime Youth Performance includes a first-week money-back guarantee. If I decide the program is not a good fit for my child, I may request a refund after the first week of training and before the start of the second week.
